Phoenix Residential Care, LLC provides 24/7 long-term group homes for individuals with intellectual/developmental disabilities, mental illness, or both. Our services encourage independence, personal growth, and community integration. Dedicated support professionals help residents with self-care, relationship building, volunteering, employment, shopping, and pursuing personal interests.
Funding Structure:

Monthly Room & Board is funded by Social Security Income (SSI/SSDI), Medicaid Special Assistance, or private pay. It covers rent, utilities, food, and household needs.

Daily Service Fee covers staffing and therapeutic support. It is authorized through the person’s local Managed Care Organization (MCO) or Local Management Entity (LME), funded by the NC Innovations Waiver, NC Medicaid, state funds, or private pay.
Levels of Group Home Care:
For individuals with intellectual and developmental disabilities:

Group Living High: For individuals with significant needs, including physical disabilities. Staff are available 24/7 (usually asleep at night).

Group Living Moderate: For those needing some daily assistance. Staff are on-site 24/7.

Group Living Low: For individuals who are mostly independent but require routine support.
For individuals with mental illness:

Group Living Moderate: Provides ongoing support and monitoring to maintain a safe living environment.

Group Living Low: Offers daily support for mostly independent residents to maintain health and safety.
